This is an island platform, running northwest/southeast, with platform 5, northbound lying southwest of platform 6, southbound.
The central reservation at the foot of the escalators to/from the intermediate level is near the northwest end, with three passages to each platform. Beyond this is the entrance, followed by the exit to the Piccadilly Line, via passage 1.
From passage 1, go down 16 then 16 steps, going northwest. Platform 5, northbound is to your left, platform 6, southbound to your right.
Turn northwest, go up 16 then 16 steps, going northwest and turn right into passage 1.
You leave the concourse going east, there are two openings into the passage separated by a pillar, the right-hand one is the one you need. The passage turns right (south), then slopes slightly down towards the escalators. There are two escalators, the right-hand escalator is normally the one you need.
The foot of the escalators is at the north end of a passage:

The second set of escalators go down southeast towards the Jubilee Line platforms.
